29 CFR 1926.451(e)(8): Direct access to or from another surface was used when the scaffold was more than 14 inches (36 cm) horizontally and/or 24 inches (61 cm) vertically from the other surface: a) Roof - On or about 8/20/13, a safe means of access was not provided for employees traversing horizontally from the 3 - 4 foot ledge of the lower level roof on the north side (face) of the building to the basket of the aerial lift. Employees accessed the basket of the aerial lift from the ledge of the lower level roof approximately 25 feet above ground level.
