29 CFR 1926.451(e)(8): Direct access to or from another surface was used when the scaffold was more than 14 inches (36 cm) horizontally and/or 24 inches (61 cm) vertically from the other surface:    a) Roof - On or about 8/20/13, a safe means of access was not provided for employees traversing horizontally from the 3 - 4 foot ledge of the lower level roof on the north side (face) of the building to the basket of the aerial lift.  Employees accessed the basket of the aerial lift from the ledge of the lower level roof approximately 25 feet above ground level.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge 6 feet or more above a lower level was not protected from falling by the use of a guardrail system, safety net system, or personal fall arrest system.    a) South west corner of roof - On or about 8/20/13, employees who performed  painting activities from the southwest corner of the lower level roof, approximately 25 feet above ground level, were exposed to fall hazards without the use of a fall protection system.
