29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening:    Location: Warehouse, 1463 Highland Avenue, Cheshire, CT 06410    The Bil-JAX Cougar Platform Lift, Model XLR-1651 was not equipped standard railing and toeboards on all exposed sides whereby the guardrail chains on the one side of the platform was equipped with a toprail chain 38 1/2 inches above the platform and the midrail chain 19 1/2 inches above the platform and did not conform to a standard railing equal in height and strength to the permanent railings provided on three other sides of the lift.    On or about 6 September 2013 an employee was injured when he fell 6 feet from a lift to a concrete floor while retrieving material from a stock shelve.
