29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about September 10, 2013, at worksite 4231 NE Park Springs Drive in Lees Summit, Missouri- Employee was using a radial skill saw to cut 5' by 8' sheet boards without using any type of eye or face protection to prevent potential eye or face injuries.   Abatement certification for this item is not required.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   On or about September 10, 2013 at worksite 4231 NE Park Springs Drive in Lees Summit, Missouri- Employees were performing roofing operations at an elevated height on a steep pitch roof without any means of fall protection by way of guardrail systems, safety net systems, or personal fall arrest systems.   Abatement certification for this item is not required.

29 CFR 1926.1053(b)(1):   Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about September 10, 2013 at worksite 4231 NE Park Springs Drive in Lees Summit, Missouri- Employees were accessing a working surface on a steep pitch roof by way of a portable ladder whose side rails did not extend at least 3 feet above the upper landing surface being accessed.     Abatement certification for this item is not required
