29 CFR 1910.134(c)(2)(i): An employer may provide respirators at the request of employees or permit employees to use their own respirators, if the employer determines that such respirator use will not in itself create a hazard.       This violation was observed on September 9, 2013; where employees were provided N95 type respirators on a voluntary basis to clean and wipe NEMA (computer) units and the employer had not determined if the respirator itself created a hazard to the employees.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 1910.134, in written or oral format to employees who wear respirators when such use is not required by the employer.     This violation was discovered on September 9, 2013 where the employer allowed employees to use N95 respirators without providing employees the basic advisory information on the respirators.
