29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about 09/01/13 at 1600 Marietta Hwy, Canton GA: the employees on the ground were observed working without hard hats where employees on the roof were throwing trash into the container below.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest:     On or about 09/10/13 at 1600 Marietta Hwy, Canton GA: the employer failed to provide fall protection for employees that were exposed to fall hazards of up to 19 feet while working on the roof installing shingles.
