29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used:  a)  At the jobsite, on or about 9/2/13:  Employees were using the upper platform of a tubular welded frame scaffold, one section high, that approximately 3 feet below the roof edge being accessed.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:(a)(LOCATION)(IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS)  a)  At the jobsite, on or about 9/2/13:  Employees were using an 8 foot step ladder to access the upper landing of a scaffold platform that was approximately 7 feet high.

29 CFR 1926.1053(b)(20): Employees did not face the ladder when ascending or descending the ladder:  a)  At the jobsite, on or about 9/2/13:  Employees were egressing the roof, using a portable ladder that was brought when the other ladder was identifed as inappropriate, and employees decended that ladder facing away from the rungs.
