29 CFR 1910.36(e)(1):  A side-hinged door was not used to connected rooms to an exit route:    Workplace, 9020 Wall Street, North Bergen, NJ    The Basement Northeast exit door where the laundry department is located leads to the East stairway, that leads to a door that opens back into the East side of the parking garage ground floor that has an exit sign indicating the path of travel is through the parking garage bay doors.  This parking garage bay door opens and closes vertically, it is not side hinged as required.     Violation observed on 9-19-13

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    Workplace, 9020 Wall Street, North Bergen, NJ      a) The Marathon  Model # RJ100-SC and Serial # 2085455 garbage container and compactor has a key operated compactor button.  This button has a key broken off inside of it, and operates whenever the button is engaged.  The device should only operate when an authorized operator with the operation button key operates the device.  This garbage container has an unlockable door to the compacting compartment approximately 30 inches by 30 inches.  An employee could suffer an amputation or death from crushing since the machine operates with the door open.     Violation observed on 9-19-13

There are other potentially safer devices to use such as the VanishPoint U-100 Insulin Syringes, however these had not been  reviewed by managers including the Bloodborne Pathogens Program Coodinator.
