29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides.  a. Second Floor/Loading Zone - On or about September 13, 2013, linen and uniforms care workers were required to lift linen accesories (hangers, garments, and wooden pallets) to the second floor by accesing an area that was inadequately guarded with a standard guardrail. Employees were thereby exposed to fall hazards of above four (4) feet to the ground level.

Cleaning Laundry Areas - On or about September 13, 2013, components on electrical equipment was not closely examined on their electrical insulation conditions. Employees were thereby exposed to electrical hazards at the following locations;  1. Northside/Soil Platform - electrical insulation was pulled away from the conveyor's power switch box's cord.  2. First Floor/ # 8 Ironer Machine - electrical insulation was loose on the power supply component.  3. First Floor/by # 10 Ironer table - electrical insulation was pulled away from its fitting on the conveyor's warning light's cord.
