(a) East Side of Roof - On or about September 10, 2013 two employees were exposed to a fall hazard to a lower level when descending from a roof using a forklift mounted platform without a guardrail system or personal fall arrest system or safety net system.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    (a) East Side of Roof - On or about September 10, 2013 three employees were exposed to fall hazard when working on roof sheathing installation on a 4:12 pitch roof with unprotected sides. Employees were not protected from falling 14 feet 2 inches to a lower level by the use of personal fall arrest system, safety net system or guardrail system.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge:    (a) Building, North Side and West Side - On or about September 10, 2013 four employees were exposed to fall hazard when ascending/descending from the building under construction using stairs which do not have stairrail system.
