29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:    (a) Jobsite:  On or about September 13, 2013, an employee was working from a ladder jack scaffold and he was not protected from falling to the ground.  The fall hazard to the employee was approximately 25 feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    (a)  Jobsite:  On or about September 13, 2013, two employees were working on a residential roof and they were not protected from falling.  The fall hazard to the two employees was about 32 feet to the ground.

29 CFR 1926.503(c)(3): When the employer has reason to believe that any affected employee who has already been trained does not  have the understanding and skill required by paragraph (a) of this section, the employer shall retrain each such employee:    (a)  Jobsite:  On or about September 13, 2013, two employees who were exposed to an approximate 32 foot fall hazard while working on a roof of a residential home, needed to be retrained so that they retain the knowledge and skill required to minimize the fall hazards while working on a roof.
