29 CFR 1926.20(b)(1): The employer did not initiate and maintain an accident prevention program as may be necessary to comply with the General safety and health provisions covered in 29 CFR 1926.20.    a) Jobsite: 233 Chestnut St, Philadelphia, PA - On or about 9/5/13, Tommy's Plumbing did not develop and implement an accident prevention program to help protect their employees from hazardous working conditions.  Employees were cutting a section of concrete out of the sidewalk measured to be approximately 4 feet in length, 3 feet in width, and 3 feet in depth.
