29 CFR 1910.212(a)(3)(ii): The point of operation of machines, whose operation exposed an employee to injury, was not guarded:     a)    In the Metal Forming and Layout Department, west side, on the 135 ton Cincinnati Press Brake, the rear of the machine lacked guarding such as but not limited to a barrier chain  to prevent employee access to the point of operation.     b)    In the Metal Forming and Layout Department, west side, on the 135 ton Allsteel  Press Brake, the rear of the machine lacked guarding such as but not limited to a barrier chain  to prevent employee access to the point of operation.
