29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c):     On or about September 13, 2013, at 2920 Lee Ave & Fairfax Ter, Little Rock, AR 72205, the employer did not ensure three employees working in a trench greater than 4 feet in depth were protected from cave-ins by a protective system. Employees were exposed to fall, falling load and caught in-between hazards.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:       On or about September 13, 2013, at 2920 Lee Ave & Fairfax Ter, Little Rock, AR 72205, the employer did not ensure employees had a safe means of egress, such as a stairway, ladder or ramp while working in a trench greater than 4 feet deep.  Employees were exposed to delay in escape, fall, falling load and caught in-between hazards.

29 CFR 1926.651(j)(2): Employees shall be protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations. Protection shall be provided by placing and keeping such materials or equipment at least 2 feet (.61 m) from the edge of excavations, or by the use of retaining devices that are sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.      On or about September 13, 2013, at 2920 Lee Ave & Fairfax Ter, Little Rock, AR 72205, the employer did not ensure employees were safe from falling/rolling materials or equipment into the trench. The equipment was placed too close to the edge of the trench; the employer did not keep the 2 feet distance required from the edge or by using retaining devices. The unprotected employees were exposed to struck by and caught in-between hazards.
