29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:     a) On or about September 11, 2013, located in the cutting area of the Beach department the employee who was assisting shear operator feeding material into shear was not adequately protected from making contact with the point of operation of the shear blade, in that the fixed guard attached to the machine did not prevent the employees hand extremities from making contact with the blade during cut cycle.
