29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access. a) Jobsite: A ladder or other safe means of access was not provided to employees working on the tubular welded frame scaffold. Employees were climbing the side frames and crossbraces to access the working levels thus exposed to fall hazards up to twenty (20) feet. Observed on or about September 17, 2013

29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6): a) Jobsite: The tubular welded scaffold was erected within ten (10) feet of the energized electrical power lines. Additionally, the scaffold was also in close proximity to the energized 240 volt conductors thus exposing the employees to possible shock or electrocution hazards. Observed on or about September 17, 2013
