29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder:      (a) Mixing rooms - On or about September 18, 2013 employees were using open sided platforms that was more than 4 feet without standard railing.

29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers shall be equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section, the width of the stair to be measured clear of all obstructions except handrails:    (a) Packing room small blender - On or about September 18, 2013 employees were using a set of stairs with four risers without standard handrails.

The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report or equivalent forms were not kept by the establishment:   (a) Site wide - On or about September 18, 2013 employees injuries were not being maintained on the OSHA 300 form or equivalent form.
