29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas. On or about September 17, 2013, at 103 Miramar Blvd, Little Rock, AR 72223, the employer did not ensure employees were working on a site cleared of scrap lumber with protruding nails and other debris. Employees were observed/exposed to cutting and puncturing of hands or other body parts on projecting nails, wire and other debris by tripping hazards due to poor housekeeping practices.

29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide: On or about September 17, 2013, at 103 Miramar Blvd, Little Rock, Arkansas 722205, the employer did not ensure employees using a job made scaffold was inspected by a competent person. As a result employees used an elevated work platform that was made of one plank less than 11.5 inches wide and greater than 10 feet long with no guard rail to prevent falling greater than 6 feet to the unfinished ground below.

On or about September 17, 2013, at 103 Miramar Blvd, Little Rock, AR 72223, the employer did not ensure employees were using fall protection systems during residential roofing work. Employees were observed/exposed to fall hazards of greater than 10 feet to finished ground below from a roof pitch greater than 6:12.
