OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: Well site: On or about September 18, 2013, and times prior to, employees were engaged in the clean out of newly drilled oil/gas well with one employee working from the rig derrick mounted racking board. The employee was provided with an emergency escape system (Geronimo) that had an inadequate anchorage. This exposed the employee to the hazard of falling and striking the ground.

This exposed them to the hazard of falling into the cellar.
