29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: On or about September 18, 2012, an employee plugged in the bin sump where the end of the cord was not equipped with strain relief and the insulated wires were exposed, exposing the employee to an electrical shock hazard.
