29 CFR 1910.22(d)(1): In every building or other structure, or part thereof, used for mercantile, business, industrial, or storage purposes, the loads approved by the building official shall be marked on plates of approved design which shall be supplied and securely affixed by the owner of the building, or his duly authorized agent, in a conspicuous place in each space to which they relate. Such plates shall not be removed or defaced but, if lost, removed, or defaced, shall be replaced by the owner or his agent.  a. Above the room where employees clean screens, boxes were stored without the load being approved and marked, thereby exposing employees to a crushed-by hazard.

29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides,  a. On the top of the screen cleaning room where employees stored boxes, employees were not protected from falls by guardrails, thereby exposing employees to a 9 foot fall hazard.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.   a. At the worksite where employees used a Workhorse Products-Freedom Automatic Screen Printing Press, the safety trip cord was not used, thereby exposing employees to a caught-in hazard.

At the workplace where employees use chemicals such as, but limited to; Screen & Press Wash, Screen Wash/Stain Remover, RhinoClean ER 8500L, RhinoClean DG 2200, and Fast Tack 383, the employer did not develop a written hazard communication program which informed employees of procedures for handling, dispensing, and storing hazardous chemicals, thereby exposing employees to a contact hazard.
