29 CFR 1910.26(c)(3)(vii): Portable ladders were used for other uses than that for which they were intended:   Warehouse Area-An employee was exposed to a fall hazard while using a metal stepladder to retrieve product from an above shelf and the spreader arms were not open or locked on or about 9/19/13.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):   Warehouse Area-Employee(s) were required to operate the two lift trucks to move product throughout the warehouse and had not completed training and/or evaluation as specified by this standard, on or about 9/19/13.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
