29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  On September 19, 2013 at the jobsite located at or near 8641 North Oak Trafficway in Kansas City, MO employees were exposed to a fall hazard in that they were working at a height over 6 feet above the next lower level without the use of an approved form of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder�s length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On September 19, 2013 at the jobsite at or near 8641 North Oak Trafficway in Kansas City, MO employees were exposed to a fall hazard in that the ladder they were using to access and egress the roof did not extend at least 3 feet above the upper landing.
