29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   a)  Jobsite located at 110 Towne Centre Drive, Pooler GA  On or about September 19, 2013, the employer allowed an employee to operate a mobile scissor lift Skyjack, Model SJ1113219, without the midrail entry chain attached, exposing the employee to a fall hazard up to 18 feet.     ABATEMENT CERTIFICATION IS NOT REQUIRED.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   a)  Jobsite located at 110 Towne Centre Drive, Pooler GA  On or about September 19, 2013, an employee used the midrail to gain height and straddle the brick wall without fall protection while working from a mobile scissor lift Skyjack, Model SJ1113219, thus exposing the employee to a fall hazard up to 18 feet.     ABATEMENT CERTIFICATION IS NOT REQUIRED
