29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    On or about 9/19/2013, at the above addressed job site, an employee was installing veneer brick on the exterior structure of a residential home and was exposed to a 7 ' fall hazard while working from a one-tier scaffold that was not fully planked.

Instance 1: An employee working from a one-tier scaffold without baseplates and mudsills.  Instance 2: An employee using a two-tier scaffold to access a roof did not have baseplates or mudsills.

Instance 2: An employee used a two-tier scaffold supported with cement block units to access a roof.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    On or about 9/19/2013, at the above addressed job site, an employee was exposed to a fall hazard of approximately 13 feet while working at a construction site.     Instance 1: An employee on a 4:12 pitch roof applying grout between the bricks without fall protection equipment.  Instance 2: An employee using a walking platform without guard rails to access the roof.
