29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: On or about September 20, 2013, and at times prior thereto, in the east shop bay #17 employees were allowed to operate abrasive wheel grinding machines where the opening between the work rest and the wheel was greater than one-eighth inch. Employees were exposed to flying fragments in the event of an exploding wheel.

29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel: On or about September 20, 2013, and at times prior thereto, in the east shop bay #7 the employer allowed employees to operate abrasive wheel grinding machines where which did not have a tongue guard provided. Employees were exposed to flying fragments in the event of an exploding wheel.
