For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall. a) Jobsite located at 105 N Anderson Dr. Hwy 80 E, Swainsboro GA - On or about, and at times prior to September 20, 2013, the employer did not ensure toe boards or other forms of falling debris protection was provided. Employees working above were using buckets, hawks, trials, spades, and mud boards, exposing employees below to struck-by hazards. ABATEMENT CERTIFICATION IS NOT REQUIRED

The fourth level uprights between the towers only consisted of crossbracing attached to the outside uprights between scaffold towers 1 and 2 and 2 and 3, exposing employees to an unstable scaffold condition. The Occupational Safety and Health Administration (OSHA), received notice through the Regional Area Office hotline system at 12:15 pm from a third party on September 21, 2013.
