29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:  a)2200 East Adams Ave, Philadelphia, PA - The employer did not conduct an evaluation of each powered industrial truck (PIT) operator's performance at least once every 3 years, thus exposing the employees to struck-by hazards. This violation was observed on or about September 6, 2013.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
