29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. a. On or about 09/19/2013, at the job site - employees were permitted to work from a 3 feet high platform placed on the second level of the scaffold, this platform elevated the employees higher than the guardrails, exposing the employees to a 17 feet fall hazard.
