(a) West Highlands subdivision, Lot #230, Atlanta, GA; The employer exposed employees to fall hazards while working approximately 16 feet above the ground from a pump jack scaffold which did not have a complete guardrail system, due to the mid-rails and end-rails not being installed.

(a) West Highlands subdivision, Lot #230, Atlanta, GA; Employees were installing siding on a residential home and were exposed to fall hazards of approximately 16 feet while working on a pump jack scaffold that was not secured at the bottom with bracing.
