29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers:  On or about September 13, 2013 and at times prior thereto, employees were working on the roof of a commercial building which had skylights and they were not provided protection against falling through the skylights.  Employees were exposed to a fall of greater than 15 feet.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about September 13, 2013, and at times prior thereto, employees were working on the roof of a commercial building with a pitch that ranged from 4:12 and up to 8:12, without using fall protection.  Employees were exposed to a fall of greater than 15 feet.
