29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:     (a) Location: 15 Harding Street, Agawam, MA: On or about September 23, 2013, two employees were observed performing roofing work on a steep roof and were exposed to a fall hazard greater than 11 feet to the ground surface below without any fall protection.       Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:     (a)  Location: 15 Harding Street, Agawam, MA. On or about September 23, 2013, employees doing roofing work were exposed to fall hazards greater than 11 feet and the employees had not been trained by the employer to recognize the hazards of falling in accordance with this subpart.
