29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.   a) Slater Mill subdivision, Lot #191, Douglasville, GA; The employer exposed employees to fall hazards while working approximately 17 feet above the ground from a ladder jack scaffold which did not have a complete guardrail system, due to the mid-rails, top-rails, and end-rails not being installed.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   a) Slater Mill subdivision, Lot #191, Douglasville, GA; The employer exposed employees to fall hazards of approximately 15 feet above the ground due to a portable ladder that was used improperly with side rails that extended 1 foot above the landing.
