29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. a) Grandview estates subdivision, lot #6, Atlanta, GA; The employer exposed employees to fall hazards of approximately 12 feet by not providing a secure ladder to access the upper landing of the scaffolding platform. Employees were installing siding on the exterior walls of the home.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. a) Grandview estates subdivision, Lot #6, Atlanta, GA; The employer exposed employees to fall hazards while working approximately 12 feet above the ground from a ladder jack scaffold which did not have a complete guardrail system, due to the mid-rails, top-rails, and end-rails not being installed. Employees were installing siding on the exterior walls of the home.
