crane crew employees were exposed to falls ranging from a height of approximately twenty to thirty feet while conducting monthly inspections of the crane trolleys, without the use of engineered anchor points, such as but not limited to, angle iron support for electrical conduits, 2" x 3" angle iron, trolley frame, end plate of the trolley frame, on the following cranes # 414 and ARL.       i) On or about 12/2/13 in the 12" Mill located at 3049 Lakeshore Road, Blasdell, New York; crane crew employees were exposed to falls ranging from a height of approximately twenty to thirty feet while conducting monthly inspections of the crane trolley, without the use of engineered anchor points, such as but not limited to, angle iron support for electrical conduits, 2" x 3" angle iron, trolley frame, end plate of the trolley frame, on the following crane # 372.      j) On or about 12/31/13 in the Center Shipping area located at 3049 Lakeshore Road, Blasdell, New York; crane crew employees were exposed to falls ranging from a height of approximately twenty to thirty feet while conducting monthly inspections of the crane trolleys, without the use of engineered anchor points, such as but not limited to, angle iron support for electrical conduits, 2" x 3" angle iron, trolley frame, end plate of the trolley frame, on the following cranes #411, # 409, #650, # 651.       k) On or about 12/1/13 in the East Shipping areas located at 3049 Lakeshore Road, Blasdell, New York; crane crew employees were exposed to falls ranging from a height of approximately twenty to thirty feet while conducting monthly inspections of the crane trolleys, without the use of engineered anchor points, such as but not limited to, angle iron support for electrical conduits, 2" x 3" angle iron, trolley frame, end plate on the trolley frame, on the following cranes # 652, #653.         l) On or about 12/31/13 in the Cooling Bed area located at 3049 Lakeshore Road, Blasdell, New York; crane crew employees were exposed to falls ranging from a height of approximately twenty to thirty feet while conducting monthly inspections of the crane trolleys, without the use of engineered anchor points, such as but not limited to, angle iron support for electrical conduits, 2" x 3" angle iron, trolley frame, end plate on the trolley frame, on the following crane # 613.       To abate this hazard in the future, the employer must ensure that fall protection devices/systems are used by employees.  Republic Steel dba Republic Engineered Products, was previously cited for a violation of this Occupational Safety and Health Standard or an equivalent standard contained in:     OSHA Inspection number 313785941, Citation 01, Item 001, Citation 01, Item 002, and Citation 01, Item 005 issued on May 16, 2011, and became final order on or about May 14, 2012, with respect to workplace located at 1807 East 28th Street, Lorain, Ohio 44055;       OSHA Inspection number 313782781, Citation 02, Item 001, issued November 4, 2010, and became final order on or about May 4, 2011, with respect to a workplace located at 1807 East 28th Street Lorain, Ohio 44055.       OSHA Inspection number 314603846, Citation 92, Item 001, issued on December 6, 2010, and became final order on or about July 7, 2011, with respect to workplace located at 2633 8th Street NE, Canton, Ohio 44704.       Pursuant to 20 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including an explanation of how these steps protect its employees from fall hazards.
