29 CFR 1926.21(b)(6)(i): Employees required to enter into confined or enclosed spaces were not instructed as to the nature of the hazards involved, the necessary precautions to be taken, and in the use of protective and emergency equipment required:    The employer does not instruct employees required to enter confined or enclosed spaces on the hazards involved in confined space entry, the necessary precautions to be taken, and on the use of protective and emergency equipment.  This violation occurred on or about September 26, 2013, at pump station #1 of the Eastside Water Treatment Plant located at 405 Long Creek Road in Sunnyvale, Texas, where two employees, who entered a dewatering pit, had not been instructed on the hazards involved in confined space entry and the necessary precautions to be taken.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    The employer does not ensure that employees on a walking/working surface with an unprotected side or edge 6 feet or more above a lower level are protected from falling.  This violation occurred on or about September 26, 2013, at pump station #1 of the Eastside Water Treatment Plant located at 405 Long Creek Road in Sunnyvale, Texas, where three employees walking near the edge of dewatering pit were not protected from falling approximately 10 feet to the lower level.

This violation occurred on or about September 26, 2013, at pump station #1 of the Eastside Water Treatment Plant located at 405 Long Creek Road in Sunnyvale, Texas, where three employees working in a dewatering pit used a straight ladder that had side rails that did not extend at least 3 feet above the upper landing surface.
