OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards:     On or before 30 September, 2013, employees were exposed to fall hazards of 20 feet while working from a pallet resting on the forks of a Crown forklift.  The pallet did not have a guardrail system nor was the wood members on the floor designed to support employees.   Additionally, the platform did not indicate the maximum work load including personnel and equipment, the weight of the empty platform, or the minimum capacity of the truck on which the platform could be used in accordance with ANSI B56.1-2009, American National Standard for Low Lift and High Lift Trucks, Section 7.37.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:       a)    North building, on the east  wall,  by the Cram-A-Lot bailer, the 220 volt electrical electrical panel box was missing the front panel that would cover the  current carrying electrical metal parts inside.

29 CFR 1910.305(b)(2)(i):     All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.     a)            North building, on the north wall by the Cram-A-Lot bailer, the 220 volt electrical junction box was missing its cover, exposing the inner electrical wiring connections.
