29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment: On or about September 25, 2013 and at other times prior thereto, at tis facility site, the employer failed to ensure an adequate written hazard assessment was conducted to determine what type of personal protective equipment was required to protect employees from the hazards of high pressure, scalding water used to clean a problematic Ultrafab scrubber.
