29 CFR 1926.416(a)(3): Before work began, the employer did not ascertain by inquiry, direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring any person, tool, or machine into physical or electrical contact with the electric power circuit.     a)  Fidelity Investor Center, 29 South Main St., West Hartford, CT. - On or about 10-10-13 - Employees were exposed to live parts of a 277 volt - 20 amp wiring system which were not de-energized or effectively capped/guarded. A contractor on site walked into live wires that were hanging down in the walking/working area and was knocked unconscious by an electric shock when his forehead contacted the exposed live wire.
