29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  At jobsite located at 297 HWY 69, Garland, KS, employees engaged in reroofing work worked from a steep roof (up to 8 in 12 sloped roof) without fall protection, exposing employees to falls of up to 8 feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  At jobsite located at 297 HWY 69, employees were not provided fall protection training to enable employees to recognize the hazards of falling and the requirements to use fall protection, when working on a roof taller than 6 feet, exposing employees to a fall hazard of up to 8 feet to the ground below.
