29 CFR 1910.269(a)(2)(i): Employees shall be trained in and familiar with the safety-related work practices, safety procedures, and other safety requirements in this section that pertain to their respective job assignments. Employees shall also be trained in and familiar with any other safety practices, including applicable emergency procedures (such as pole top and manhole rescue), that are not specifically addressed by this section but that are related to their work and are necessary for their safety.     (a) Jobsite - On or about October 08, 2013 the employer did not provide safety training to employees that work near energized power lines.

However, only one trained person need be available if all new employees are trained in first aid, including CPR, within 3 months of their hiring dates.     (a) Jobsite - On or about October 08, 2013 employees were not trained on CPR.

29 CFR 1910.269(r)(1)(i): Before an employee climbs, enters, or works around any tree, a determination shall be made of the nominal voltage of electric power lines posing a hazard to employees. However, a determination of the maximum nominal voltage to which an employee will be exposed may be made instead, if all lines are considered as energized at this maximum voltage.     (a) Jobsite - On or about October 08, 2013 employees were trimming tree limbs near electrical power lines. The employer did not determine the nominal voltage of the single phase 7620 volt electric power lines.

29 CFR 1910.269(r)(1)(iv): Branches that are contacting exposed energized conductors or equipment or that are within the distances specified in Table R-6, Table R-9, and Table R-10 may be removed only through the use of insulating equipment.   Note: A tool constructed of a material that the employer can demonstrate has insulating qualities meeting paragraph (j)(1) of this section is considered as insulated under this paragraph if the tool is clean and dry.     (a) Jobsite - On or about October 08, 2013 employees used an uninsulated Stihl pole pruner to trim tree branches within three feet of an engerzied electrical power lines. The employer did not maintain the minimum approach distance from a 7620 single phase voltage load electrical power line.
