One of the two flexible cord sets observed on the site was damaged. This condition exposed employees to electrical shock hazards.

The flexible cord that was used to power the fan was missing the ground pin. This condition exposed employees to electrical shock hazards.

The outer insulation was ripped along a short length of the cord set and thereby, exposed the inner conductors. This condition exposed employees to electrical shock hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:              a) Dizdar Brothers Sewer & Construction Co., Inc. @ 1001 Pine St., St. Louis, MO 63101: On and before 9/30/13, the employer did not ensure that the flexible cord set that was used to power the forced air fan was free from damage. The strain relief for the female-end of the cord was damaged, so that the outer insulation was pulled away from the plug and thereby, exposed the inner conductors. This condition exposed employees to electrical shock hazards.

Employees utilized a 14ft ladder for access to a 14ft excavation. This condition exposed employees to a fall hazard of 14ft while entering and exiting the excavation.
