The flexible cord that was used to power the fan was missing the ground pin. This condition exposed employees to electrical shock hazards.

The outer insulation was ripped along a short length of the cord set and thereby, exposed the inner conductors. This condition exposed employees to electrical shock hazards.

The strain relief for the female-end of the cord was damaged, so that the outer insulation was pulled away from the plug and thereby, exposed the inner conductors. This condition exposed employees to electrical shock hazards.
