29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  On or about 10/03/2013, at the above addressed jobsite, an employee was not protected from falling through holes (skylights) when replacing (skylights) plastic panels on the roof at approximately 12 feet above ground level without a cover, or guardrail system, exposing him to a fall hazard.  NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT MUST BE PROVIDED FOR THIS ITEM.
