29 CFR 1926.451(b)(1): 29 CFR 1926.450(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports:    Job site located at 8116 Grant Street, Omaha, NE 68118-The employer and employees worked on a working level of a scaffold that was not fully planked or decked between the front uprights and the guardrail supports. There were two levels on the scaffold that were worked on to install the roof joists and the cross beams.  The employer stated that they were working from the lower level of the scaffold installing the cross beams the day of the inspection.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  Job site located at 8116 Grant Street, Omaha, NE 68118-The employer or employee who are considered the competent person on the site did not inspect the scaffold that they were working on to identify defects or hazard such as, but not limited too, not having all working levels of the scaffold fully decked or planked before employees are allowed to work on it.
