Employees are exposed to falls and struck by hazards while being lifted on a scaffold platform approximately 12 from a lower level, and the employer did not ensure that the operator had successfully completed forklift operator's training noted in paragraph (l) of 29 CFR 1910.178.

29 CFR 1926.20(b)(1):     The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:    Employee working in an approximately 12 foot high pic board which was mounted on the forks of a Gehl Forklift, Model GRs6-42 was exposed to fall  hazards and the employer did not initiate and maintain an accident prevention program to control and eliminate the hazards present at the jobsite in accordance with the OSHA regulations.

The forklift used was not specifically designed to support scaffold platforms.
