A) Workplace, on or about October 18, 2013, employees operated a Daewoo forklift that did not have a seatbelt, exposing employees to a hazard of being thrown out of the operators seat of the forklift.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a hazard of being struck by the swing arm of an excavator with a grapple attachment. A) Processing area, on or about October 18, 2013, employees operate a Samsung excavator with a grappler attachment that did not have a functioning horn, exposing employees to a struck by hazard.
