Employees were exposed to a fall hazard of approximately 20ft to the ground below; on or about 10/22/13.  b)  Worksite, 3056 Shore Rd. Bellmore NY - Employees were exposed to falling approximately 12ft while framing a residential home.  The employees were working around unguarded window openings; on or about 10/22/13.  Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
