29 CFR 1926.501(b)(5): Each employee on the face of formwork or reinforcing steel was not protected from falling 6 feet (1.8 m) or more to lower levels by personal fall arrest systems, safety net systems, or positioning device systems:    a. On or about October 22, 2013, on the top of the form, two employees performing masonry operations were not protected from a 16 feet 2-inch fall hazard by the use of a guardrail system, safety net system or personal fall arrest system.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed    a. On or about October 22, 2013, an employee used a 12 foot A-frame Louisville step ladder improperly, in that the ladder was being worked from in the closed position, exposing the employee to a 10 foot 3-inch fall hazard.
