29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system:   (a) Location: 702 Southbridge Street, Worcester, MA: On or about October 22, 2013, two employees were observed performing work on a low-sloped roof without fall protection and were exposed to a fall hazard greater than 15 feet to the ground surface below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) Location: 702 Southbridge Street, Worcester, MA: On or about October 22, 2013 two employees working on a commercial roof with no fall protection were exposed to falls up to 15 feet and had not been provided fall hazard training in accordance with this subpart.
