The employee was not utilizing face and/or eye protection while dry cutting bricks to various sizes to prevent debris, brick, and mortar particles from flying back into the employee's face, on or about 10/22/13.

The Townes of Avondales Subdivision, Brochenhurst Drive & Gravel Springs, Buford, GA - Employees were exposed to caught-between hazard.  Employees were operating the portable concrete mixer without having the guard covering the rotating parts (drive pulley and belt). Employees were using the concrete mixer for the brick laying process of the newly constructed townhome, on or about 10/22/13.

The Townes of Avondale Subdivision, Brochenhurst Drive  & Gravel Spring Drive, Buford, GA - Employees were exposed to a chemical hazard.  The employer did not develop and implement a written hazard communication program where employees were exposed to hazardous chemicals/substances on the jobsite.  Employees were using Coosa masonry mortar cement mix, which contains Portland cement when mixing up the dry concrete mix for the brick laying process on the newly constructed home, on or about 10/22/13.

The Townes of Avondale Subdivision, Brochenhurst Drive & Gravel Spring Drive, Buford, GA - The employees were exposed to chemical hazard.  The employer did not maintain the safety data sheets for the cement masonry mix that is used on the jobsite for the employees use.  Employees were using coosa masonry mortar cement mix, which contains Portland cement, when mixing up the dry concrete/cement for the brick laying process on the newly constructed home, on or about 10/22/13.
