29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  On or about October 22, 2013 at the above addressed jobsite, employees performing residential roofing operations approximately 8.7 feet above the ground were not protected from falls, thereby exposing employees to fall hazards.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.    On or about October 22, 2013 at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
